Programs

In order to accomplish our Goal of improving patient health and quality of life we have developed 2 distinct programs to address your needs:

Patient Financial Assistance - Patients who cannot afford the co-pay associated with their insurance plans, and who meet program guidelines, can have their co-pays paid for by the Fund. No chronic disease sufferer should be without access to the medications they need. In addition to receiving financial assistance, all patients will be provided access to our therapy management portal; DiseaseTrak.com.
