Therapy Management

The Chronic Disease Fund has partnered with the Chronic Disease Management Group, Inc. (CDMG) to provide therapy management services. Therapy management is an integral part of our program ensuring patients the greatest outcome from their prescription drug therapy.

CDMG provides therapy management through an online patient portal. Upon acceptance into one of our programs, the patient will be required to complete a patient profile and provide us with required additional information. Every month thereafter the patient will complete a short list of questions in order to assist the patient in monitoring their disease progression. This information is contained in a secure environment in compliance with HIPAA guidelines.
