Disease Overview

The Chronic Disease Fund provides financial assistance to patients diagnosed with chronic or life-altering disease. Our funding is designated and donated by CDF defined Disease States. CDF exclusively defines both the disease state and the FDA approved specialty therapeutics that are included using our proprietary guidelines.
