Corporate Disclosures
In the interest of full disclosure, and in support of our commitment to corporate values and ethics, we must disclose the following:
The Chronic Disease Fund is a tax-exempt, 501(c)(3) charitable organization. Our Board of Trustees, in accordance with IRS guidelines, votes and approves all significant contracts, especially those where a conflict of interest may exist.
The Chronic Disease Management Group, Inc. (CDMG) provides the therapy management portal DiseaseTrak.com that the Chronic Disease Fund uses as its accounting and tracking system as well for providing patients with therapy management. Michael Banigan is the founder and majority shareholder of this for-profit entity. To date, (February 13, 2006) CDF has not paid CDMG any money, in accordance with the contract terms, for the use and implementation of the system. As such, the Chronic Disease Fund is able to achieve the highest direct-to-patient-assistance levels among its charitable foundation peers.
